Vision Checklist

This checklist is comprised of the most common signs and symptoms of a binocular vision disorder.  They include typical behaviors, observable functional deficits and indicators of poor cognitive processing.  Completing this checklist is often the very first step a family takes toward discovering the existence of our unique services as well as the possibility of a treatable visual component to their child's learning or behavior issues.  When applicable, we encourage the classroom teacher to also complete a checklist on behalf of the child. Often teachers are good observers for binocular vision disorders that interfere with school work.

Call Our Office

Every patient is evaluated in a step-by-step process which starts with an initial phone call to determine if our services are appropriate for you or your child.  This first phone call starts with several questions about your visual concerns, covers the various areas of vision and visual processing that are evaluated, and also includes information about insurance and financial options.  After this in-depth conversation, our staff will recommend scheduling a binocular vision evaluation if the signs and symptoms addressed align with our field of specialized care.  You may call our office at 812-482-1411.

Extended Health Questionnaire

Our questionnaires are very detailed and allow Dr. Bauernfiend to review many aspects of patient history before you arrive for evaluation.  Included are sections covering developmental milestones, diet, medical history, school environment and most importantly a section to describe your child as a person and provide any other information that would be important in our treatment of your child.  If you are unsure which of our questionnaires is appropriate, please call our office.  Dr. Bauernfiend and the Vision Development Center team are here to support your child and your family throughout the process of evaluation and potential treatment.
